The back of the patient invoice gathers information that the patient may need to convey to Labcorp's Billing Department.

The top section can be used to submit or update the patient's personal information. This information can also be submitted or updated by calling 800-845-6167, by sending a fax to 866-227-2939, or by logging into your Labcorp patient account. Labcorp will use this information to file a claim with the insurance company.
