Chronic conditions dashboard

The chronic conditions dashboard enables administrators and care management teams to rapidly generate targeted patient lists. By highlighting overdue quality metrics, the dashboard helps support proactive outreach and patient care.

Coding assessment dashboard

The coding assessment dashboard highlights discrepancies between biochemical test results and ICD-10 codes, pinpointing patients needing assessment for newly detected or rapidly progressing chronic kidney disease (CKD) and diabetes, enabling timely intervention for frequently miscoded tests.

It breaks down CKD stages as follows: Stage 3a (65.9%), Stage 3b (23.3%), Stage 4 (7.9%), and Stage 5 (2.9%). The ICD-10 coding distribution shows 2,241 coded cases and 6,519 uncoded cases. An agreement matrix compares laboratory results with provided ICD-10 codes, detailing the number of cases per CKD stage. The report includes options for different views: Report View, Provider View, and Patient View.

Community health dashboard

The community health dashboard allows FQHCs to advance health equity by providing deidentified lab data insights on diabetes and CKD across vulnerable populations. With interactive maps and filters by ZIP code, payer and Area Deprivation Index (ADI), users can uncover geographic and population-level disparities, supporting targeted interventions and resource allocation. The Insight Analytics care gap summary brings together both internal and external Labcorp results, helping FQHCs identify and close care gaps with greater accuracy. With flexible views by patient, provider or ZIP code, it enables teams to add previously performed test results directly into the electronic medical record (EMR), supporting smarter population health strategies.

Diagnostic Assistant™

Diagnostic Assistant simplifies test ordering with a powerful global search that captures Labcorp tests as well as those from other labs, making it easier for FQHC providers to find what they need. Integrated electronic add-ons through select EHR systems streamline workflows, helping clinics manage care efficiently without needing to call customer service.

Gaps in care program

The gaps in care program offers convenient, at-home kits for key quality measures tied to conditions like colorectal cancer, diabetes and CKD. Designed for individual orders or as part of broader gaps in care initiatives, these kits meet Labcorp’s high diagnostic standards while helping FQHCs improve care access and quality performance. We also offer online patient scheduling for blood draw appointments at our nationwide network of patient service centers.

Open orders program

Our open orders program helps FQHCs boost patient compliance and improve quality measure performance by sending timely, personalized email reminders to patients who haven’t completed their lab testing. 

 

 

19% With up to 19% of patients completing testing within 11 days of a reminder, the program supports follow-through on important care while giving providers visibility into impact through a dedicated dashboard.
