Chronic conditions dashboard

The chronic conditions dashboard enables administrators and care management teams to rapidly generate targeted patient lists. By highlighting overdue quality metrics, the dashboard helps support proactive outreach and patient care.

Coding assessment dashboard

The coding assessment dashboard highlights discrepancies between biochemical test results and ICD-10 codes, pinpointing patients needing assessment for newly detected or rapidly progressing chronic kidney disease (CKD) and diabetes, enabling timely intervention for frequently miscoded tests. 

A coding report for chronic kidney disease (CKD) from Labcorp. The report includes a summary showing a CKD coding rate of 27% and an accuracy rate of 69%. It breaks down CKD stages as follows: Stage 3a (65.9%), Stage 3b (23.3%), Stage 4 (7.9%), and Stage 5 (2.9%). The ICD-10 coding distribution shows 2,241 coded cases and 6,519 uncoded cases. An agreement matrix compares laboratory results with provided ICD-10 codes, detailing the number of cases per CKD stage. The report includes options for different views: Report View, Provider View, and Patient View.

Community health dashboard

The community health dashboard allows FQHCs to advance health equity by providing deidentified lab data insights on diabetes and CKD across vulnerable populations. With interactive maps and filters by ZIP code, payer and Area Deprivation Index (ADI), users can uncover geographic and population-level disparities, supporting targeted interventions and resource allocation.
